New Patient Information
Your First Visit
As our patient, your goals and desires come first. Dr. Locke and our staff will cater your first visit to your needs. Patients who haven’t seen a dentist in a while may need a comprehensive exam, including X-rays, intraoral photos, and a TMJ analysis. Patients with good oral health often visit us for a cleaning on their first visit. Our main goal is to ensure your comfort and address any pertinent issues. From there, we can create a plan to achieve your long-term oral health goals.
New Patient Forms
We have made our new patient forms available to you as Adobe Acrobat files. For your convenience, we recommend printing and completing these forms at home. Please remember to bring your completed forms with you for your first appointment. If you have any problem getting these files to print properly, please call our Woodway dental office and we will gladly send or fax them to you.

Insurance and Financing
Dental insurance provides valuable benefits to our patients, and we want to help you maximize your coverage. Our front office staff will file all necessary forms and send any required documents to your insurance provider. However, Dental insurance is limited and your dental needs may exceed your dental coverage. Cosmetic and restorative treatments are generally not covered by insurance. Dr. Locke strives to make dentistry affordable for all of his patients. We accept cash, personal checks, and credit cards (AmEx, MasterCard, Visa and Discover). Patients can also make low monthly payments on treatments through CareCredit or Allegro Credit. These trusted names in healthcare financing make the process simple and provide flexible payment plans that suit any budget.
